This form was created using Google forms. The teacher this survey was created for requested that it collect responses from students that complete the statement "Be...". She was then going to use these responses to create a bulletin board. For the survey, I created eight questions. The first two questions were short answer for their first and last name. The next question was multiple choice for what block(class) they are in. These questions were all required. After this I added some more instructions for filling out the "Be..." statements, and following this there is five short answer responses for completing the statement. The teacher wanted at least one response from each student, so I made one question required. The next four responses were optional and not required, so the survey could be submitted even if these questions are left blank. Google forms also allows the user to change the theme, so I changed it to an lab setting background to make it more fun and ascetically pleasing.

In the survey I created, the teacher will likely learn some attributes that are important to students. A teacher could also use Google forms to create an interest survey, an "About me" survey, or even to schedule tutoring and parent teacher conferences. The survey would have the student select some time slots that the teacher has available. It could also have a section for students to fill out if they would be willing to help tutor a peer. This could help ensure that students are getting needed help even with hectic schedules.

This tool encourages teachers to be designers, facilitators, and analysts. The teacher would be a designer because they are using Google forms to create a personalized learning experience. They are a facilitator, as they are creating a learning experience that fosters creative expression and communicating ideas. Lastly, the teacher is an analyst when they look at student responses and decide how to respond. Depending on how the tool is used, I believe it can help teachers fulfill many of the ISTE standards for educators.

I enjoyed using this tool because it is so versatile. There are options to make your questions short answer, paragraph, multiple choice, time, scale, grid, check boxes, and more. This allows teachers to get answers that are fitting to any question they may desire to ask. The option to make or not make questions required also helps to guide responses. I felt that this tool allowed the user to collect the exact information desire, and it was fairly easy to create and us. I really enjoyed all the different theme options that allowed me to personalized my survey. I foresee myself frequently using this tool in my future classroom.
